If you are relying upon insurance to pay for medical services, documentation of your insurance coverage, including your insurance company’s name and your group and policy numbers.

• License:

Your current, valid driver’s license.

• Referrals:

Any referral to this office that your insurance company requires.

• Medical Records:

Medical records from other healthcare providers that are relevant to your visit with us. If you don’t have them, we can help you obtain it.

• History:

Please refer to the information under “New Patients” above to learn how to easily and securely provide your personal and medical history in advance of your visit via our online Patient Portal, thereby helping to ensure that you are seen as quickly and efficiently as possible.
