New Patients
Our practice uses a state-of-the-art electronic medical record (EMR) to store information regarding your health, including your health history prior to joining our practice.
To save time at your first visit and ensure that your health needs are fully met, we ask that you complete your medical history before your first appointment and update it before all subsequent visits. We have made this very simple! When you call to make your first appointment, you will be given a unique password. With that and your email address, you can then log on to the Patient Portal website with assurance that all the information you provide will be encrypted and secure, in full compliance with the federal Health Information Portability and Accountability Act (HIPAA).
With Patient Portal, you will have the opportunity to:
- Provide information such as your address, phone number and your Social Security number;
- Complete a medical history;
- Send messages to request appointments, clarify billing issues and ask general questions of the staff;
- Request prescription refills;
- View a schedule of upcoming appointments; and
- See an up-to-date “Health Record” containing lab results, vital signs, medications and active health issues.
All the information you provide will be imported into your medical record in our offices, ready for you when you arrive for your appointment. And you can update your information at anytime.
Oh, and should you ever misplace your password, don’t worry. Just give us a call and we’ll issue you a new one.
If for some reason you are unable to use Patient Portal, you may print out the Healthcare History Form and complete it at your convenience before coming in for your first visit.
